The Brotherhood Janko is run by a human named Giovanni. They are a “lesser evil” compared to some of the other gangs in Torch, and do not deal in slaves. They are primarily involved in extortion and drugs. Two other members include Jennifer Mosley, the woman who guided the party to the Brotherhood when they first arrived in Torch, and Lenny.

The party agreed to bring a letter from Giovanni to Badurth at the Festhall of Falling Coins. The party then aided them and their two rival gangs, The Kindred of Yoj and The Fire Kings, in taking down the (Black Lotus affiliated) Gray Orb. Afterwards, Giovanni double crossed the two recently allied gangs, and was nearly assassinated, but escaped death with help from the party. The other two gangs then besieged the Brotherhood in the dungeon of the Black Lotus under the Gray Orb's headquarters. The Brotherhood prevailed with the help of the party, who from that time forward received a cut of Giovanni's business (15%) in return for their aid (18, 19, 20).

More recently, Helga Slaughter and the Army in Ysgard helped The Brotherhood when they were being extorted for money by Tiamat's Chosen.
